Please bring the following to your appointment:
Photo ID
Insurance Card
Any previous X-rays
Any medical reports and labs you have
If your visit is for a hip or knee problem, please bring shorts or loose fitting clothing to help facilitate your visit
If you have an HMO insurance
You must make sure that a referral from your primary care physician is in our office before your appointment. We suggest you call our office 1 -2 days before your appointment to check on your referral. Patients with managed care will not be seen without their referral.
